Open to the Public! Location: Midtown CHC - 60 E 3750 S, South Salt Lake, UT 84115 If you are 40 and up and have not had breast imaging in the last year, book your 3D screening mammogram and/or dense breast ultrasound on The Boob Bus! We will be parked at the Community Health Clinic in Midtown. We accept most insurance plans for mammography. We are in-network with Select Health, Blue Cross/Regence, PEHP Summit Plan, U of U Health Plans, and more. Exclusions include EMI, Tricare, PEHP Advantage, and a few others. We are not yet in network with UHC or Cigna and out of network deductibles may apply. Call 866-747-2662 for details. If you are not yet 40, you may still qualify for breast imaging and/or may wish to pay out of pocket for your baseline evaluation. The self-pay rate for a 3D mammogram is $299 and a dense breast ultrasound is $399. Note that this is for SCREENING only (if you have a lump, breast pain, or other concerns you should speak with your provider and schedule a diagnostic exam). **If you are pregnant or nursing/breastfeeding, a screening mammogram is not recommended.
