Location: Core Injury Clinic 2222 W 3500 S West Valley City, UT 84119 If you are 40 and up and have not had breast imaging in the last year, book your 3D screening mammogram and/or dense breast ultrasound on The Boob Bus in collaboration with Core Injury Clinic. We accept most insurance plans for mammography. We are in-network with Select Health all plans, Blue Cross/Regence, PEHP Summit Plan, Optum, AARP Medicare, and more. Exclusions include EMI, Tricare, PEHP Advantage, and a few others. We are not yet in network with UHC commercial and out of network deductibles may apply. Call 866-747-2662 for details. If you do not have insurance, you may qualify for a free mammogram: https://cancer.utah.gov/do-i-qualify/ If you are not yet 40, you may still qualify for breast imaging and/or may wish to pay out of pocket for your baseline evaluation. The self-pay rate for a 3D mammogram is $329. Note that this is for SCREENING only (if you have a lump, breast pain, or other concerns you should speak with your provider and schedule a diagnostic exam). **If you are pregnant or nursing/breastfeeding, a screening mammogram is not recommended.
