Location: LifeVantage 3300 N Triumph Blvd Lehi, UT 84043 If you are 40 and up and have not had a screening mammogram in the last year, book your 3D screening mammogram with us! Mammograms are the best way to detect cancer early, while it is most treatable. If you are under 40 and at a higher-than-average risk for breast cancer, you may still be eligible (call or text us at 866-747-2662 to discuss). We accept most insurance plans for mammography. We are in-network with Select Health all plans, Blue Cross/Regence, PEHP Plans, Optum, AARP Medicare, and more. Exclusions include EMI, Tricare, and a few others. We are not yet in network with UHC commercial and out of network deductibles may apply. Call or text 866-747-2662 for details. **Note that these appointments are for screening ONLY (if you have a lump, breast pain, or other concerns you should speak with your provider and schedule a diagnostic exam). **If you are pregnant or nursing/breastfeeding, a screening mammogram is not recommended, but you may be able to have an ultrasound if you are empty.
