Location: Amatoria Wellness, 1244 N Main St, Tooele UT 84074 If you are 40 and up and have not had breast imaging in the last year, book your 3D screening mammogram and/or dense breast ultrasound on The Boob Bus in collaboration with Amatoria Wellness! We accept most insurance plans for mammography. We are in-network with Select Health, Blue Cross, PEHP Summit Plan, Optum/AARP Medicare Advantage, Cigna and more. Exclusions include EMI, Tricare, PEHP Advantage, and a few others. Transportation fees may apply, depending on your plan and deductible. We are not yet in network with UHC commercial plans and out of network deductibles may apply. Call 866-747-2662 or visit the "Insurance & Payment" page on our website for details. If you are not yet 40, you may still qualify for breast imaging and/or may wish to pay out of pocket. The self-pay rate for a 3D mammogram is $329. Note that this is for SCREENING only (if you have a lump, breast pain, or other concerns you should speak with your provider and schedule a diagnostic exam). **If you are pregnant or nursing/breastfeeding, a screening mammogram is not recommended.
